Referral Eligibility: Anyone can refer an eligible person to the program (with their permission) including: Self-referral, carers, family or friends, health professionals. For people who live with severe mental illness which cause some difficulties in your everyday life, are Aged 18+ years, live in South Western Sydney local government areas of: Bankstown, Fairfield, Liverpool, Campbelltown, Camden, Wollondilly & Wingecarribee and are not receiving funding under the National Disability Insurance Scheme (NDIS)
